KCICN also assists individuals transitioning from psychiatric or substance use disorder inpatient treatment programs find housing and obtain temporary housing subsidies through the Housing and Recovery through Peer Services program. The HARPS team can help participants find housing as well as connect with community supports, such as behavioral health treatment, primary care, and employment services. Housing subsidies are available for temporary rental assistance, overdue rent, security deposits, housing application fees, reasonable storage costs, reasonable moving costs and other housing related expenses.

After returning to the US, he built upon the passion for empowering others in his work as a job coach for adults with disabilities at PROVAIL. It was at PROVAIL that he first became acquainted with Community Homes and their mission to provide exceptional housing to adults with disabilities. In addition to the housing described above, KCICN contracts for specialized housing services known as Residential Services. These residential environments are licensed boarding homes and residential treatment facilities. They are available for adults who are not yet ready to live on their own and need daily supervision. The facilities are staffed 24-hours-a-day seven days a week and provide room and board, housing stabilization services, and medication monitoring.

Care coordinators monitor service plans during the year, following up regularly with clients and service providers to ensure that their situations have stabilized. Alyssa has over 20 years of experience in community-based care for adults with developmental disabilities. Prior to her current position, she was the licensed care provider for one of Community Homes’ houses, providing 24-hour residential care for five residents. She joined the Community Homes administrative team as a program coordinator in 2008 and was promoted to director in 2017. She holds a BA in Psychology and Business Management from Whitworth College and a certificate in management from Open University in Leeds, England. Community Homes was established in 1995 to provide affordable and supportive housing for adults with intellectual and developmental disabilities in King County.

If the client isn't financially eligible, notify social services. Social services will state fund Fast Track services when the client isn't financially eligible during the fast track period. Authorize payment for NF care if the client is both functionally and financially eligible.

The Long-Term Care Ombudsman is a trusted resource in mediating complaints or concerns you may have about anyone living in an adult care facility (e.g. adult family home, Assisted Living Facility, nursing home). The Family Caregiver Support Program provides help and resources for those who provide unpaid care to a senior adult or and adult with a disability. Adult Day Health programs includes the core services mentioned above plus skilled nursing services, skilled therapy services (e.g., physical therapy, occupational therapy, or speech therapy), and psychological or counseling services. Tom Bartholomew’s son Sam, 27, has an intellectual disability due to a congenital brain malformation. Luckily, Sam’s family was introduced to Community Homes when he was in high school.

The determination of Fast Track is ultimately up to social services. Authorize in ACES for in-home or residential HCB waiver if the client is both functionally and financially eligible. Screen all clients to determine potential for HCB services. For in-home service applicants, discuss the food assistance program and inquire if the household would like to apply for food benefits.
